2025.03.17: Xen Host was successfully restarted and put back into service at 2025-03-16 16:49:00. No hardware issues were detected.
Around 16:40:00 on 2025.03.16 a prodxo.oit.ncsu.edu Xen host failed and restarted. VMs on the hosts with HA enabled were restarted on alternate Xen hosts.
Staff are investigating and will attempt to provide a list of VM's impacted.

The OIT Production vCenter instance (vcenter.ncsu.edu) is currently having issues communicating with the 2FA proxy service. This is resulting in users experiencing an "Invalid Credentials" error upon attempting to log in. We have identified the source of the problem and are working to resolve it as quickly as possible.
Running VMs remain fully operational and fully protected from hardware failures (HA)--only access to the guest consoles, etc. is impacted. Users needing to perform critical operations can contact OIT_PLATFORM until the issue is resolved.

At 12:00pm (noon) the OIT vcenter will be rebooted to address a performance issue. The reboot may take up to 20 minutes. No VMs will be impacted. Users will not be able to access vcenter interface during the reboot.

VM's with Windows update KB5022842 and secure boot enabled may lose their boot drive on reboot. This can be repaired while the system is powered down by disabling secure boot in vm settings. VMware has released an update which is being tested and should enter production during the spring.

The issue appeared to have started around 2:15am, due to the zabbix monitoring server being impacted it was unknown at the time.
A PCS rack switch became unresponsive and a restart resolved the issue. A vendor support request is being opened to determine the root cause.

OIT vCenter is online. Phase 1 completed, MFA enabled in non-enforcement mode for user accounts not yet enrolled.
On 1/9/23 at 6 p.m.
OIT vCenter will be reconfigured to use the Duo LDAP proxy in bypass mode. During this phase, MFA will be enabled however NOT enforced. Meaning if your .admin account is not yet enrolled in Duo, logins will still be allowed. This change will require a brief outage for a reboot.
On 2/13/23 (tentative)
OIT will plan to change from bypass mode to enforce 2FA mode. Meaning that vCenter users’ .admin accounts will be required to be enrolled in Duo to manage your VMs at the console through https://vcenter.ncsu.edu. This date may be extended based on Duo .admin account enrollment processing and number of users not already enrolled.
To register your <dot>admin accounts in Duo by the requested timeline, please put a ticket into oit_security@help.ncsu.edu in sufficient time to avoid interruption for your vCenter access.

Logins are either succeeding immediately, are delayed then successful after a few minutes, or just failing. This authentication issue also caused backup related failures overnight. Update: Two additional ESXi hosts have been impacted and included in affected list.
At 4:40pm 2 ESXi hosts became unresponsive causing VMs to restart on other ESXi hosts in the cluster. Platform engineers are working with Comtech to determine the root cause. A list of impacted VM's will be provided shortly.
