Duo reported a resolution to the issue at the end of the day yesterday and all systems at NC State appear to be operating normally again.
--- Update 10:29 am 8-21-2023 ---
Duo has identified the issue causing authentication slowness and failures to load the Duo Prompt. They are currently working on a solution.
--- Original Post 9:00 am 8-21-2023 ---
Users may experience login errors and messages during authentication to NC State Services. This includes anything behind Shibboleth or other services integrated with Duo such as Azure AD (Office 365) etc. In some cases waiting and retrying the login will allow it to go through.
Duo is aware of the issue and is working on correcting it.

Resolved 12-15-2021 10:55 am
User logins through Shibboleth and Duo should be back to normal. Duo posted a status update to their service citing a service disruption in AWS and has taken initial steps to restore connectivity with their systems. OIT is continuing to monitor our systems and we see no additional activity indicating continued problems.

We are currently investigating an issue with Duo authentications that may add a significant delay to logins through shibboleth or can result in not being able to log in at all. We hope to identify the cause and will provide an update as soon as we know more.

DUO is reporting that their telephony provider has resolved the issue and SMS delivery of codes should be operating normally.

Original Post
There is a known issue with DUO's SMS Delivery. The Help Desk has reports it may have started for some users as early as yesterday. DUO is currently investigating the reported problem.
Users will want to use an alternative 2FA method for DUO other than SMS if available. Do not try to repeatedly send more SMS text codes if the first few attempts do not go through. Continued failed attempts will result in account lockouts.
We strongly encourage users to set up the Duo Mobile App which is the preferred method for DUO 2FA. Setting up multiple methods ensures you have options when one method may not be working. If you need assistance with setting up the Mobile App or with accessing your account please contact the NC State Help Desk at 919-515-4357(HELP) or visit https://help.ncsu.edu.
